If you are a new patient to our office, please arrive a few minutes early to register. Information regarding your insurance and health history will be necessary to complete the registration form.
In the event you are unable to keep your appointment, kindly notify the office 24 hours in advance.
FEES
Our fees are set at a reasonable level based upon the community standard. Please feel free to discuss your charges with us if a question arises.
We participate in several insurance programs including Medicare. Payment is required at the time of service unless you belong to one of these programs. Many plans require a co-payment which is requested at the time of your visit.
TELEPHONE CALLS
Many times a question arises outside the office visit. A simple telephone call may be all that is necessary to handle the problem. We encourage you to call us anytime.
PRESCRIPTIONS AND REFILLS
Prescriptions are generally written with a specified number of refills. In the event you need additional refills, please have your pharmacist call our office.
REFERRALS AND CONSULTATIONS
If you have been referred to us by another physician for consultation, please let us know. A report will be sent directly to your doctor regarding your condition.
